Patient
and Family Satisfaction Survey Instructions and Information
Patient Satisfaction surveys are to be given to the patient two
weeks after the initial hospice visit. Family
satisfaction surveys should be given to family members
two months following the death of the patient.
NAHC
will receive surveys directly from patients and their
families, and will provide the following quarterly report to
NAHC members or subscribing agencies:
- How many surveys you mailed (based on your response)
- How many surveys NAHC received by patients and families
- The percent of participants responding to each question
on a scale of 1-5 (strongly disagree to strongly agree)
- What the average responses were for each questionfor
your hospice compared to all hospices participating in
the survey
Your agency will be responsible for
costs associated with photocopying, distributing, and providing
postage for surveys.
To participate in the surveys, please
visit www.nahc.org/hospicesurveys and
provide us with your information. We will then email you
the survey within 2 business days.
