BeyondNow are calling for nominations for the
Excellence in Innovation Award.

Thank
you for your interest in Excellence in Innovation Award, presented by Cerner
BeyondNow.
The
2009 winner will be announced at the Opening General Session
of the 28th Annual
Meeting & Exposition, Sunday, October 11, at 3:00pm, in Los
Angeles, CA.
The goals of the award are as follow:
- To acknowledge and reward individuals, home care organizations,
or hospices that develop innovative programs that expand
upon the traditional role of home careas
part of the continuum of health care services;
- To identify and promote examples of innovations in
home care from which other organizations
can learn; and
- To inspire creativity and encourage further innovation
in the home care field.
Highlights from the 2008 Cerner Award Presentation
The
Excellence in Innovation Award
winner was announced at Annual Meeting in Fort Lauderdale,

At the opening session of the 27 th Annual Meeting
of the National Association for Home Care & Hospice in Ft. Lauderdale,
FL, President Val J. Halamandaris announced the winner
of the first annual Excellence in Innovation Award, Baptist
Health Home Health Network from Little Rock, AR. Making
the presentation was John Landis, senior vice president
for Client Works at Cerner BeyondNow, the award’s
sponsor.
Announced earlier this year, the award showcases
successful programs and service delivery models that advance the practice
of home care and hospice and the role of these vital services in the nation's
health care system.

Award Winner Baptist Health
Home Health Network distinguished itself by creating a program that grew out of
an original planning session, "20/20 Solutions." The strategy took on
the growing incidence of chronic illness within the city of Little Rock
and the associated costs it had on that community as well as the employees
of Baptist Health System. This led to the creation of an original home
health chronic care model emphasizing best practices that facilitate
patient engagement and self-management behaviors, and prepares the organization
to function as a proactive team member in coordinating the care of the
patients, families and physicians. This model places the home health
agency at the hub of the overall health care process.
Expressing her thanks for the award, Baptist
Home Health Administrator, Beth Hennessey said, "Throughout our journey
to get to this point, we've been repeatedly reminded of the vivid message
of Mother Teresa--that we are instruments in God's hand, and he writes
beautifully. Our greatest joy in receiving this award would be for others
in the home care industry to join us on this journey. Pick up your pen
and help us write the next chapter in the history of home care and hospice."
"They have a unique program for hospice that
relates to dementia care — what they're doing is truly remarkable," Halamandaris
said. Third place went to Partners Home Care from Boston, MA, which came
up with an expansive program in wound care.
