Welcome to the Call for Presentations Submission

We are delighted that you have elected to submit a proposal for consideration for the educational program for the 6^th Annual Private Duty Home Care Leadership Summit of the Private Duty Homecare Association. This year's meeting will take place at the Westin Long Beach in Long Beach, California from January 23-25, 2011.

Submission deadline: July 12, 2010

We have prepared an easy to use checklist to assist you with your preparation for the submission of your proposal. The checklist identifies all of the information required for your submission, and it provides guidelines and requirements to help you prepare this information prior to starting the online submission process.

When you are ready to start the online submission process follow this link. Remember that once you begin your submission, you can not stop and return to it later. It must be completed in its entirety. Please gather all of the information on the checklist prior to starting.

Key items to remember when submitting your proposal:

• 'Don't go Back' - Do not use your browser's back button during the process. You will have an opportunity to make corrections via email at the end of the process.
• 'Print as you go' - You may choose to print each page as you progress through the submission process. There is a print button on the bottom of each page so that you can refer back to previous entries if necessary.
• 'Review at the End' - You will be given an overview of your entire submission at the end of the process as well as the assigned Proposal ID number for use in all correspondence.
