NAHC Opposes Proposed Home Health Copays and Inflation Update Cuts in President’s 2015 Budget

The National Association for Home Care & Hospice (NAHC) strongly opposes the Obama Administration’s proposed home health copayments. NAHC maintains that deficit reduction should not come in the form of a “sick tax” on the nation’s poorest, sickest, and most vulnerable individuals.

“Essential home health services are at risk,” said NAHC President Val J. Halamandaris. “Previously enacted changes will cut Medicare spending on home health services by more than $100 billion over the next ten years — while less than $20 billion is spent each year. As a result of these cuts, Medicare will pay home health agencies less than their costs, meaning that 41 percent of all Medicare home health agencies will be under water in 2014, and 60 percent by 2017. Congress should therefore reject any additional cuts and any home health copay whether the reason is postponement or elimination of scheduled cuts in physician fees or deficit reduction.”

The President’s budget also includes a 1.1 percentage point reduction each year in the Market Basket Index (inflation) updates for post-acute providers including home health care from 2015 through 2024. The proposed updates would reduce reimbursements by a total of nearly $98 billion. If they take effect, these payment reductions would be in addition to the 2014 rebasing of home health and home health productivity adjustments that lower payment rates by over 14 percent starting in 2015.

Hospice Association of America and State Hospice Associations Urge Revision to Hospice Vaccine Billing Policy

In May 2013 the Centers for Medicare & Medicaid Services (CMS) issued Change Request (CR) 8098 under which, effective October 1, 2013, Medicare systems would reject claims for vaccines provided to hospice patients by non-hospice providers.  CR8098 also reiterated existing CMS policy that hospices could only bill influenza, pneumococcal, and hepatitis B vaccines provided to hospice patients if the hospice enrolled as a Part B supplier. 

In response to concerns raised by the National Association for Home Care & Hospice’s Hospice Association of America (HAA) and others, CMS recently issued CR 8620, under which, effective April 7, 2014, it will reverse the edits put in place by CR 8098.

CMS’ issuance of CR 8098 raised additional questions within the hospice provider community relative to the requirement that hospices may only bill for preventive vaccines if they are enrolled as a Part B supplier.  On February 20, 2014, HAA and 40 state associations representing home care and hospice providers sent a letter to CMS requesting that it reconsider the policy requiring hospices to become Part B suppliers in order to bill vaccines.

Private Duty Nursing Supervisor

We have an opening for a Private Duty Nursing Supervisor in St. Louis, MO. Must have current Missouri RN license, at least two years of private duty nursing experience with medically fragile patients, and one year of private duty supervisory experience. The position is full time, Monday through Friday.

Responsibilities include:

- Creates Plan of Care. Documents patient status in an accurate, timely and concise manner and reviews all documentation for accuracy and completeness
- Competent and knowledgeable in performing baseline-nursing skills. Knowledge in wound care, tube feedings, tracheotomy care, catastrophic injury care, pulmonary care, ventilator care, spinal cord injury care, seizure disorder care, catheter care, and diabetes care strongly encouraged
- Communicates effectively with patients, families and physicians and other team members
- Demonstrates organization and leadership abilities by effective use of time and personnel through supervision and teaching.
- Maintains a committed and cooperative attitude with staff, promoting teamwork and harmony by readily assisting other staff and patients.
- Supervises employee at recerts, and supervises each employee annually and PRN.
- Maintains Recert/Evaluation calendar
