President’s Budget Includes Provisions for Home Health Copays, Additional Payment Cuts for Post-Acute Care Providers
Budget includes other proposals – bundling and value-based payments – impacting home health care

President Obama released his 2015 budget proposal earlier this week, laying out his agenda and legislative priorities for the coming year. Two of the provisions included in President Obama’s FY15 budget are similar to previous proposals that would be of great concern to the home health community. For more on NAHC’s position on these budget proposals, please see NAHC Report, March 5, 2014.

Specifically, President Obama’s budget once again calls for the introduction of a home health copayment - as well as further cuts to post-acute care providers, including home health care.

President Obama’s budget also includes provisions for a bundled payment program for post-acute providers, as well as value-based purchasing for post-acute care providers. The specifics of these four proposals are included below:

CMS Provides Clarification on Rescission of Hospice CR 8273: New CR Anticipated with October 2014 Effective Date

On November 7, 2013, the Centers for Medicare & Medicaid Services (CMS) issued Change Request (CR) 8273. CR8273 instructed Medicare Administrative Contractors (MACs) to implement a new edit to deny hospital inpatient claims when there is a Medicare hospice claim for the same beneficiary within the same covered period and the principal diagnosis code on the hospital inpatient claim is an exact match to any of the hospice claim’s diagnosis codes.  The scheduled effective date was April 7, 2014.  In late February, CMS issued a rescission of Medlearn Matters article 8273, which accompanied CR 8273. 

The National Association for Home Care & Hospice’s Hospice Association of America (HAA) has learned from CMS that the rescission was necessary so that an edit related to the CR could be revised.  It is CMS’ expectation that the rescinded CR 8273 will be reissued as CR 8625 at some time in the near future. It will have an effective date in October 2014. This means that rejection of inpatient hospital claims that include one of the hospice diagnoses as the principal diagnosis will not begin in April as previously scheduled, but will begin in October instead.

Private Duty Nursing Supervisor

We have an opening for a Private Duty Nursing Supervisor in St. Louis, MO. Must have current Missouri RN license, at least two years of private duty nursing experience with medically fragile patients, and one year of private duty supervisory experience. The position is full time, Monday through Friday.

Responsibilities include:

- Creates Plan of Care. Documents patient status in an accurate, timely and concise manner and reviews all documentation for accuracy and completeness
- Competent and knowledgeable in performing baseline-nursing skills. Knowledge in wound care, tube feedings, tracheotomy care, catastrophic injury care, pulmonary care, ventilator care, spinal cord injury care, seizure disorder care, catheter care, and diabetes care strongly encouraged
- Communicates effectively with patients, families and physicians and other team members
- Demonstrates organization and leadership abilities by effective use of time and personnel through supervision and teaching.
- Maintains a committed and cooperative attitude with staff, promoting teamwork and harmony by readily assisting other staff and patients.
- Supervises employee at recerts, and supervises each employee annually and PRN.
- Maintains Recert/Evaluation calendar

- Record intake from prospective clients
- Obtain availability from nursing and aide field staff
- Coordinate private duty clients' needs with medical staff availability
- Assign nurses and aides to service patients
- Communicates effectively with patients, families and medical staff
- Maintains a committed and cooperative attitude with patients and staff

Job Requirements:
- Minimum of one year experience in private duty nursing scheduling
- Great interpersonal and telephone skills
- Strong organizational, time-management and computer skills
- Ability to multi-task
- Must be detail oriented and pleasant
