Senate Republican Leader Reconfirms his Support for Home Care and Hospice

Senate Republican Leader Mitch McConnell (R-KY) recently reconfirmed his support of home care and hospice. During a home health rally in his home state of Kentucky, Mr. McConnell said of home care, “[it’s] one of the stars of our health care system… What’s not to like about home health care?”

Senator McConnell further emphasized his support for those who work in home care and hospice by declaring that working in home care is a, “noble calling” and that if he becomes Majority Leader, the issue of Medicare home care and hospice cuts will, “be on the schedule and we’ll be voting on it.”

The Republican Leader’s remarks came at a rally in Louisville against cuts for home health care agencies that will affect around 21,000 Kentuckians over the next four years. Nationwide, NAHC’s own analysis has found that home care cuts through rebasing would make Medicare margins for home healthcare drop to a negative (9.77%) while further estimating that 72.9% of all home healthcare agencies will be operating below cost by 2017. These cuts would pose a significant risk to access for needed home health services throughout the country.

Prior to his recent comments, Senator McConnell has emphasized his support for the home care and hospice community when he addressed attendees to NAHC’s  2013 Annual Meeting.

CMS Extends Exceptional Circumstance Exemption for Hospice Employment of Nurses

In a October 3 Survey & Certification letter, S&C 15-01-Hospice, the Centers for Medicare and Medicaid Services (CMS) extended the period of time for hospices to qualify for an “extraordinary circumstance” exemption when they believe that the nursing shortage has affected their ability to directly hire sufficient numbers of nurses. The policy will remain in effect until September 30, 2016 and CMS will re-evaluate the policy prior to that time. 

The exemption is only applicable to the employment of nurses, not social workers or counseling services - i.e. chaplains - which are other core hospice services.  The exemption allows hospices to contract for nursing personnel instead of having to employ them directly. In cases where the hospice would like to use the exemption, they must notify their state agency.  This agency will maintain the notification and necessary accompanying information as well as validate the need for the exemption during surveys.
