CMS Issues Face-to-Face Documentation Template

As home health agencies continue to struggle to understand and apply the new documentation requirements under the physician face-to-face encounter rule, CMS has unveiled a form for voluntary use that may help. The form is based on an Electronic Health Record (EHR) project within CMS and the Office of the National Coordinator. It is a slimmed-down revision of the earlier EHR version and covers only the F2F elements rather than a full scale clinical assessment of the patient.

The template is available in two versions: electronic and paper. These draft templates can be accessed by following the links below.



CMS will be holding Open Door discussions of these draft templates. However, they can be used now by home health agencies. CMS intends the templates to be for purely voluntary use and will not mandate that physicians or HHAs use them. CMS intends the completed form to be part of the physician’s record, thereby fulfilling the requirements of the new rule that took effect on January 1.

A Reminder about CMS’ Upcoming Migration from IACS to EIDM

NAHC wants to remind its members that CMS is migrating the P S & R application from IACS to EIDM, effective February 9.

During a transition period, running from January 30 – February 8, users will not be able to create new IACS accounts, and any changes to existing IACS accounts will not be carried over to the new system. It is anticipated that P S & Rs may be inaccessible for some time during the transition period but that time frame is not known as yet.

Given this change, it is strongly recommended that existing users log on to their IACS accounts and change their passwords prior to close of business on Friday, January 30 to ensure that they are current, that they haven’t expired, that they won’t expire during the transition period, and are known to the appropriate users. This will eliminate the need to secure additional assistance with a password change once the transition to EIDM is complete.

2015 NAHC Annual Meeting & Exposition — Call for Presentations
Proposal deadline extended to Monday, February 9, 2015

NAHC is extending the deadline to submit proposals for its 2015 Annual Meeting and Exposition. The National Association for Home Care & Hospice invites prospective faculty, speakers, and presenters to submit presentation proposals for the its Annual Meeting & Exposition – held Wednesday, October 28 – Friday, October 30, 2015 at Gaylord Opryland Resort & Convention Center in Nashville, TN – by February 9, 2015.

All members, including associate members and vendors, are encouraged to participate as faculty in the 2015 Annual Meeting. NAHC’s policy is that at least one faculty member in each proposed program must be an active employee of a home care or hospice provider and that member will serve as the designated primary faculty for the session.

The Annual Meeting educational sessions are offered on October 29-30, 2015 to address the needs of home health, hospice, and private duty providers, as well as clinical professionals including nurses, therapists, and physicians.

NAHC will again make it possible for potential faculty, speakers, and presenters to submit presentation proposals online.  Click here to submit online.

All submissions must be received by Monday, February 9, 2015. No submissions or change in content on previously submitted presentations received after this date will be considered.
