Description:

Social determinants of health care is a growing area of focus throughout the health care industry. This is true for home care (both skilled and unskilled) and hospice. It is an area that affects both clinical and financial aspects of operations.

This webinar will provide an overview of the current status of the effects and experiences of two organizations actively engaged in this key area. Attendees will gain greater knowledge on how social determinants may affect the delivery of care and financial performance of their organizations as well as ideas that may be utilized.

The presenters are Rose Madden-Bayer and Anthony Evans. Rose is the Senior Vice President, Population Health and Clinical Support Services of the Visiting Nurse Service of New York. Anthony is the President of Pure Healthcare, Centerville, Ohio. Both have extensive experience in social determinants affecting our segment of the industry. Attendees will benefit from their wealth of knowledge and experience.

The webinar is offered by NAHC and HHFMA through its Innovations Committee.
